Your registration process can be expedited if these forms are printed and completed prior to your first visit to one of our offices. In addition to these forms, please bring your proof of household income, proof of guardianship if applicable, as well as your commercial insurance, Medicaid or Medicare card.

Payment Options
We realize that your ability to pay for health care is an important factor in your decision to seek our services. Consequently, to the greatest extent possible, we seek to be of service regardless of the ability to pay. We do this by accepting a range of payment sources, as well as, assisting persons lacking financial resources to apply for assistance.

Participants Needed
What: A study to determine if a lifestyle intervention or a lifestyle intervention plus Metformin helps children and teens prevent or reverse weight gain caused by certain medications.
Who: Children and teens ages 8-17 years who have a mood disorder, are overweight, and are currently taking a second-generation anti-psychotic or may start taking them.
Where: Visits take place in the patient's mental health clinic during their regularly scheduled appointments.
Payment: Compensation up to $155 for patients and their caregivers may be available for time and travel
